Social Worker

This position leads the front-line of Engager's Kidney Disease Education (KDE) Platform. You will give patients and their families the valuable knowledge and support needed to live successfully with Chronic Kidney Disease. In addition to delivering the KDE curriculum directly to patients, you will act as a resource to the coordinated care team and help identify potential barriers to care and opportunities for additional intervention.

Qualifications:

  • Master's of Social Work Degree

  • Active Clinical Social Worker License

  • At least two years of work experience in a Chronic Illness Education and Support role

  • Technical skills including experience using Microsoft Office/Google Productivity Suite 

  • Self-motivated with strong relationship skills

Job Description and Summary

As an Education leader, your main responsibilities will be to deliver the Kidney Disease Education curriculum to patients, both in-person and through Engager's telehealth platform. You will also provide input on how to improve the curriculum to better serve patients.  You will administer Patient Assessments, which measure the effectiveness of Knowledge Transfer, and participate in calls and ongoing communication with the managing physician and/or staff.​

Dietitians

This position is responsible for counseling patients who have been diagnosed with CKD. The Dietitian role will focus on designing and delivering KDE curriculum modules covering nutrition management for kidney disease patients (CKD), how adhering to a "CKD Diet"can slow the progression of CKD and how a patient can prepare for a transition from CKD to End-stage Renal Disease.

Qualifications:

  • completed a college/university educational program in dietetics and/or nutrition that is accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND). The program can be either a Coordinated Program in Dietetics or a Didactic Program with a Dietetic Internship

  • have passed the Commission on Dietetic Registration's Registration Examination for Dietitians

  • have a current dietitian license/certification in at least one state 

  • At least two years of work experience in patient-facing role

  • Technical skills including experience using Microsoft Office/Google Productivity Suite 

  • Self-motivated with strong relationship skills

You will work with our Product Management team to design and build beautifully simple tools that deliver life-changing knowledge to patients learning to live with Chronic Kidney Disease. This is an assignment for leaders looking to leave their mark on the Healthcare Industry.

Medical Scribe -  remote positions available 

Scribe

Engager's medical scribe team is at the center of our patient and provider engagement platform. You will collaborate with our solutions team and our clients to deliver an engaging telehealth experience. This is a remote position that includes opportunities for career advancement as well as paid training curriculum.

Qualifications:​

  • High school diploma, GED, or equivalent

  • Bachelor’s degree with pre-health career track preferred

  • Knowledge of medical terminology and human anatomy

  • Experience using EHR software (EPIC or Cerner preferred), word-processing programs, and database software programs

  • Minimum of 60 Words-Per-Minute typed

  • Strong administrative, writing, and verbal communication skills

Job Description and Summary

The medical scribe team at Engager works to quickly prepare medical record documentation and charting for clients. As a member of this dynamic team, you will remotely attend telehealth sessions with providers and patients. You will be responsible for capturing and transcribing the discussion between the patient and provider while updating the medical record documentation in EHR applications.

 

Day-to-day tasks will include:

  • Ensures medical record compliance by self-documentation and attestation.

  • Updates patient history, physical exam charts, and other pertinent information in the patient EHR

  • Prepare and send documentation to clients for review and approval by following client and facility-specific procedures.

  • Monitor basic lab results and screening procedures.

  • Perform other duties and tasks to improve client productivity and clinical workflows

  • attend internal team meetings
